Start here — what happened to you?
Pattern 01 — Billing Fraud
YOU'RE NOT IMAGINING IT.

Unexpected, inflated, or completely fabricated charges are one of the most documented patterns across HCA facilities. This isn't a billing mistake — it's a pattern.

What others have reported
Being charged for procedures, tests, or medications they never received
The same charge appearing multiple times on a single bill
Procedure codes on their bill that don't match what the doctor ordered
Facility fees that were never disclosed before treatment
Medicare and Medicaid billed at rates far above what was actually provided
Documented and on record

47 cases documented across 17 states. Billing discrepancies have been filed as exhibits in federal court proceedings. If you were overbilled, your experience may qualify as a whistleblower case under the False Claims Act — which means you may be entitled to a portion of any recovery.

Pattern 02 — Record Manipulation
THE RECORDS
TELL A DIFFERENT STORY.

If your medical records don't reflect what actually happened during your care — or look different each time you request them — this is one of the most serious patterns we've documented.

What others have reported
Requesting records months apart and getting different versions each time
Clinical notes that don't match what was said or done during their stay
Entries that appear to have been added after they were discharged
Being denied access to the audit log — the record of who changed what and when
Diagnosis codes on discharge that don't match what the doctor told them
Documented and on record

54 cases documented. Three federal sanctions motions filed. Forensic comparisons of portal records versus certified discovery productions have been filed as court exhibits — showing specific changes made after discharge at Southern Hills, Sunrise, and MountainView hospitals in Nevada.

Pattern 03 — Improper Discharge & Transfer
SENT HOME
TOO SOON.

Federal law requires hospitals to stabilize any patient with an emergency condition before discharging or transferring them. This law is being violated.

What others have reported
Being discharged while still in pain, unstable, or with unresolved symptoms
Being transferred to another facility without clear medical explanation
Being turned away at the emergency room or triaged out quickly
Discharge timing that seemed connected to their insurance status rather than their condition
Returning to the ER within days of discharge with the same or worsening condition
Documented and on record

13 cases documented across 6 states. EMTALA — the federal Emergency Medical Treatment and Labor Act — requires stabilization before discharge or transfer. Violations carry individual hospital liability and are reportable directly to CMS.

ADD YOUR EXPERIENCE.

If you or someone you care for was sent home too soon, your account matters.

Pattern 04 — Standard of Care Failures
THE CARE
WASN'T THERE.

Hospitals are legally required to provide a standard of care. Chronic understaffing is not an excuse. It's a documented pattern — and a liability.

What others have reported
Waiting hours for a nurse to respond to a call button
Receiving the wrong medication or the wrong dose
Falling while in the hospital due to lack of supervision or assistance
Developing pressure injuries (bedsores) during their stay
Feeling like staff were too stretched to provide basic care
Documented and on record

28 cases documented across 12 states. Documented care failures correlate with HCA's publicly reported nurse-to-patient ratios and internal staffing records. Understaffing that causes patient harm is grounds for negligence claims.

Pattern 05 — Discrimination & Civil Rights
YOUR RIGHTS
DON'T STOP
AT THE DOOR.

Federal law prohibits discrimination in any healthcare setting that receives federal funding. HCA receives billions in federal funding annually. The law applies.

What others have reported
Being denied an interpreter or accessible communication despite requesting one
Feeling their pain or symptoms were taken less seriously than other patients
Being refused a reasonable accommodation for a disability
Receiving different care or faster discharge than patients with better insurance
Staff behavior that felt racially or culturally dismissive
Documented and on record

11 cases documented across 6 states. ADA Title III and Section 1557 of the Affordable Care Act both apply. Complaints can be filed directly with HHS Office for Civil Rights — a federal agency with enforcement authority over HCA.

ADD YOUR EXPERIENCE.

Discrimination is underreported. Your account helps establish the pattern.

Pattern 06 — Privacy & HIPAA Violations
YOUR HEALTH
INFORMATION
IS PROTECTED.

HIPAA gives you specific rights over who can see your health information. Those rights are being violated.

What others have reported
Their diagnosis or treatment details shared with family members without permission
Medical information disclosed to employers or insurance companies
Staff discussing their case in public areas or with unauthorized people
Requests for their own records ignored or delayed beyond the legal 30-day window
Being unable to get their records at all
Documented and on record

8 cases documented across 6 states. HIPAA complaints are filed with HHS Office for Civil Rights. Unauthorized disclosure and records access violations are among the most straightforward cases to document and report.

How This Data Is Built
EVERY NUMBER ON THIS PAGE IS SOURCED.
This database is built from direct patient submissions, federal court filings, regulatory complaints, and public records. Every pattern is cross-referenced against at least two independent sources before it is added. We do not estimate, extrapolate, or inflate. When a case is documented here, it means a real person reported a real experience and we have the record to back it up.
